La Porte City Nursing & Rehabilitation Center has provided service to the community since 1966. Over the years, the health care services provided there have grown and changed to meet the changing needs of area seniors. In its nearly 50 year history, the facility has also experienced changes in its name, from Colonial Manor to La Porte City Nursing & Rehab Center. As of October 1, 2015, the facility has changed once again, to La Porte City Specialty Care. The name change has been made to align better with the spectrum of services provided at the center. La Porte City Specialty Care remains a part of Care Initiatives, an Iowa–based, not-for-profit organization for over 25 years.
Along with the new name comes a new mission statement, logo, slogan and overall brand. Care Initiatives’ mission statement very concisely reflects its purpose: “Improving quality of life for Iowans and their families during life’s health transitions through compassionate, individualized care.”
Care Initiatives pinwheel logo represents the continuum of care with four main quadrants representing rehabilitation services, skilled nursing care, long-term care and hospice care. The intersecting lines represent how these services intertwine as the needs of the facility’s residents change. The blending of the lines and colors represents life’s health transitions; the calming blue and green colors represent comfort and health.
Complementing the change of name and logo is a new slogan. “For All of Life’s Health Transitions” sums up the facility’s core purpose, helping people transition through life’s many health issues as a community-based facility. While many things are changing in the representation of Care Initiatives and La Porte City Specialty Care, the values and unique health care services remain the same.
So don’t be surprised when you call and a voice answers “La Porte City Specialty Care.” Be assured the updated brand is supported by the same people serving the needs of the La Porte Citycommunity.
